TREATMENT, PAYMENT, AND HEALTH CARE OPERATIONS

Project Recovery San Diego may use and disclose your protected health information (PHI) for treatment, payment, and healthcare operations, including but not limited to:

• Sharing test results with other healthcare providers to confirm a diagnosis;

• Providing your diagnosis or other health-related information to your insurance provider or billing service to obtain payment for services rendered;

• Reviewing your information as part of our quality improvement initiatives.

In compliance with legal guidelines, Project Recovery San Diego may also use or disclose your PHI for the following purposes:

• Providing health-related information to you;

• Contacting you regarding appointments, Project Recovery San Diego updates, or other healthcare-related services;

• Incidental disclosures, such as listing your name on a sign-in sheet;

• Compliance with laws, including reports of suspected abuse, neglect, or violence;

• Providing necessary information to law enforcement or correctional institutions;

• Sharing information with coroners, medical examiners, funeral directors, or organ procurement organizations;

• Public health activities as requested by a public health authority or the FDA;

• Responding to health oversight agencies;

• Compliance with court orders, subpoenas, discovery requests, or other lawful processes;

• Research activities;

• Averting a serious threat to health or safety;

• Military, veterans affairs, national security, intelligence, Department of State, or presidential protective service activities;

• Providing necessary information to public or private disaster relief agencies;

• Disclosing information to a family member, relative, or close personal friend for:

  • Notification of your location, general condition, or death;

  • Assisting with healthcare needs, such as picking up prescriptions or understanding follow-up care instructions.

AUTHORIZATION FOR OTHER USES

Project Recovery San Diego will not use or disclose your PHI for any other purposes without your written authorization. You have the right to revoke any authorization at any time by submitting a written request.

YOUR RIGHTS REGARDING THE PRIVACY OF YOUR HEALTH INFORMATION

Subject to legal limitations, you have the following rights regarding the use and disclosure of your PHI:

• Request restrictions on certain uses and disclosures (though Project Recovery San Diego is not obligated to agree to all requests);

• Receive confidential communications regarding your PHI;

• Inspect and obtain copies of your PHI, subject to certain exceptions;

• Request amendments to your PHI;

• Receive an accounting of disclosures of your PHI;

• Obtain a copy of this Privacy Notice.

Project Recovery San Diego has the following legal responsibilities concerning your PHI:

• We are required by law to maintain the privacy of PHI and provide you with notice of our legal duties and privacy practices.

• We are required to abide by the terms of this Privacy Notice while it is in effect.

• We reserve the right to change our privacy practices and apply those changes to all PHI we maintain. Any revised Privacy Notice will be posted in our office and available upon request.
