Project Recovery San Diego will not use or disclose your PHI for any other purposes without your written authorization. You have the right to revoke any authorization at any time by submitting a written request.

Subject to legal limitations, you have the following rights regarding the use and disclosure of your PHI:

Request restrictions on certain uses and disclosures (though Project Recovery San Diego is not obligated to agree to all requests);

We reserve the right to change our privacy practices and apply those changes to all PHI we maintain. Any revised Privacy Notice will be posted in our office and available upon request.
